Here's my rule for if someone is a capital-D designer: 

If you have a portfolio, you're a Designer. 

It's the one thing that I think every would-be Designer has. Most
people who create flyers for their kids' soccer team don't keep a
copy around for posterity sake. =) It's probably an overly
simplistic rule, but it works for me.
