I'm a bit late to this thread but when I was going through grad school and working on my own portfolio, I kept a list of portfolios that I liked on del.icio.us:

My list is all over the board - some are more suited for visual designers, while others are more focused on UX. In quickly glancing through the list again, there are very few that describe/illustrate their process and project outcomes.
When I was applying for full time jobs last spring (and internships the spring before that), most employers looked at my portfolio and brought it up during the interview. When I interviewed for my current job, I was even asked to give an hour-long portfolio presentation.
Having sat through several portfolio presentations myself in the past year, I'm amazed by the number of portfolios and presentations that don't actually follow the basic principles of user experience. It's also easy to show a few screenshots/wireframes/storyboards but actually describing the design process, justifications for design, and project outcomes is a much much harder skill (IMHO).
